Connecting You to Care and Resources
Helping You Navigate Healthcare and Community Services
At Two Rivers Public Health Department (TRPHD), our Community Health Workers (CHWs) help individuals and families connect with healthcare, community resources, and services that support healthier lives.
We understand that finding healthcare, understanding available services, and knowing where to turn for help can sometimes be challenging. Our Community Health Workers are here to help you take the next step.
What Happens When You Contact a CHW?
- Tell Us What You Need: A Community Health Worker will listen to your concerns and help identify services or resources that may be helpful.
- Explore Your Options: Together, you can discuss available programs, community resources, and possible next steps.
- Get Connected: Your CHW can assist with referrals, appointments, and navigating services to help you access the support you need.
